> A High is a trend from the watch's sensors, not a measurement, so treat it as a reason to get checked and not as a diagnosis. Google says a blood pressure report needs at least five days of wear in September and insulin resistance at least seven.

Health Guardian’s first monthly reports reached Pixel Watch 3, 4 and 5 owners on October 1, and the results did not line up with everyone’s tests. [Gadgets & Wearables reports](https://gadgetsandwearables.com/2026/10/01/pixel-watch-health-guardian-reports-confusing-results/) that several early users got a High insulin resistance trend despite glucose or A1C tests they say were normal, and that others questioned a high blood pressure trend after a more reassuring reading from a conventional cuff. It was not all misses: one owner with type 2 diabetes said the watch correctly showed high insulin resistance, and another with pre-diabetes got the same result.

[Android Police](https://www.androidpolice.com/first-google-health-guardian-update-landed-correct-diagnosis/) saw both outcomes in one household. A reviewer with a history of blood pressure trouble got High for insulin resistance and for blood pressure, while the reviewer’s partner, on a Pixel Watch 3, got an all-clear on all three measures. The reports are sparse, and some users are asking Google for the data behind the label. The watch doesn’t measure blood pressure in mmHg or blood glucose. Google’s models read pulse, movement, sleep and other data the watch already collects, so what you get is a trend and not a reading.
