Is this the first Apple Watch with it?
No. It comes with watchOS 27, which reaches Series 9 and later on September 14. Apple lists the Advanced Cycle Tracking app with perimenopause and menopause support on the Series 12 spec sheet and on the Ultra 4's. The Series 12 ships with it rather than being the only watch that has it.
